accessing premium channels on set top boxes

When will we be able to access premium (paid subscription) channels on set top boxes? It's a hideous waste to spend $120 a month on Dish, Comcast, or DirecTV to get HBO and Showtime in 1080P, but what choice is there when that is the content that one wants?
